Job Summary

A company is looking for a Care Manager, Registered Nurse (Remote).

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
• Develop proactive care plans to enhance members' overall wellness and address identified issues
• Utilize clinical tools and data to conduct evaluations and consult with interdisciplinary teams to overcome barriers

Required Qualifications

• Active and unrestricted Registered Nurse (RN) License in Alabama or compact licensure in state of residence
• 3+ years of clinical experience
• 2+ years of case management, discharge planning, or home healthcare coordination experience
• Associate's Degree in Nursing (required); Bachelor's Degree (preferred)
• Certification as a Case Manager is preferred
