Job Summary

A company is looking for an RN Case Manager to manage a panel of members with complex medical and behavioral needs remotely in Arizona.

Key Responsibilities

• Engage members to complete comprehensive needs assessments addressing medical, behavioral, and socioeconomic needs
• Develop and implement person-centered care plans to manage chronic health conditions and promote wellness
• Collaborate with internal teams and community resources to implement care plans and support member self-management

Required Qualifications

• Current, unrestricted independent licensure as a Registered Nurse in Arizona
• 4+ years of clinical experience
• 1+ years of experience with MS Office applications
• Must reside in Arizona
