Job Summary
A company is looking for a Care Manager Transition of Care.
Key Responsibilities
- Evaluate member needs through post-discharge assessments and medication reconciliation
- Develop care/service plans in collaboration with healthcare teams to support member transitions
- Facilitate education and resources for members and coordinate services among healthcare providers
Required Qualifications
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
- 2 - 4 years of related experience in care management
- Licensure such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
