Job Summary

A company is looking for a Cardiac Check-In Health Coach.

Key Responsibilities

• Perform comprehensive health assessments and monitor patients' progress in cardiac rehabilitation
• Provide personalized care planning and education on cardiovascular health and lifestyle modifications
• Collaborate with a multidisciplinary team to ensure comprehensive care and maintain accurate documentation

Required Qualifications

• Bachelor's or Master's degree in Health, Exercise Science, Kinesiology, or a related field
• CPR and First Aid certification
• Experience in a clinical or rehabilitation setting, ideally with cardiac patients
• Background in fitness, health coaching, or exercise physiology is highly desirable
• Motivational and goal-oriented approach to patient care
