Job Summary
A company is looking for a Clinical Consultation Pharmacist.
Key Responsibilities
- Conduct telephonic consultations with patients regarding specialty medication and disease state management
- Perform medication reconciliation, screen for drug interactions, and provide side effect management recommendations
- Interpret prescriptions, contact prescribers for clarifications, and ensure compliance with internal and external regulations
Required Qualifications
- Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree
- Current, unrestricted pharmacist license in state of residence
- At least two years of experience as a practicing licensed Pharmacist preferred
- Strong clinical background required
- Experience and/or understanding of Specialty Pharmacy medications desirable
