Compact RN Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop ongoing care plans and collaborate with providers to address members' unmet needs
• Provide education to members and families on disease processes and healthcare options

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
