Florida Licensed Nurse Case Manager

Job Summary

A company is looking for a Case Manager RN (Remote).

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
• Develop proactive care plans to enhance members' short and long-term health outcomes
• Conduct assessments using various data sources and collaborate with interdisciplinary team members

Required Qualifications

• Active and unrestricted RN licensure in Florida or compact licensure in state of residence
• 3+ years of clinical experience
• 2+ years of case management, discharge planning, or home healthcare coordination experience
• Associate's Degree in Nursing (required); Bachelor's Degree (preferred)
• Certification as a Case Manager (preferred)
