Health Navigator

Job Summary

A company is looking for a Health Navigator to support patients in a chronic care remote monitoring program.

Key Responsibilities

• Monitor and screen incoming alerts, calls, and chat communications
• Conduct patient onboarding and schedule audio-visual calls with patients
• Report patient concerns to the Registered Nurse and assist with technology troubleshooting

Required Qualifications

• High school diploma or GED required
• Experience in a clinical setting, preferably in a patient-facing role
• Familiarity with electronic health records and patient documentation
• Strong computer skills and ability to learn new technology
• Ability to work in a HIPAA compliant environment and handle stressful situations
