Health Support Coordinator

Job Summary

A company is looking for a Health Support Coordinator to provide support within the Care Management team.

Key Responsibilities

• Manage and respond to Utilization Management, Case Management, and Disease Management requests
• Maintain customer service excellence during all interactions with members and providers
• Assist with workflow processes and provide referrals to health coaches for member outreach

Required Qualifications, Training, and Education

• 2+ years of related experience
• Bachelor's or Associate degree preferred
• Experience in a call center environment is advantageous
• Knowledge of Medicare and Medicaid is a plus
• Familiarity with claims processes and BCBSMN products preferred
