Job Summary

A company is looking for a Telephonic RN Nurse Care Manager.

Key Responsibilities

• Manage a caseload of high-risk patients, coordinating care and addressing barriers
• Collaborate with multidisciplinary teams to develop and implement individualized care plans
• Conduct clinical assessments and ensure timely interventions based on patient needs

Required Qualifications, Training, and Education

• Active R.N. License
• 2 or more years of direct home health experience or care management
• Ability to perform extensive telephone assessments
• Knowledge of Medicare regulations and home care standards
• Self-motivated and organized with the ability to meet deadlines
