Licensed Clinical Case Manager
Job Summary
A company is looking for a Clinical Case Manager Behavioral Health DSNP.
Key Responsibilities
- Conduct comprehensive assessments of members' needs and eligibility for services
- Perform crisis intervention and coordinate care with clinical providers
- Develop care plans utilizing clinical judgment to address member issues and improve outcomes
Required Qualifications
- 3 years of direct clinical practice experience post-master's degree
- Knowledge of mental health and substance abuse disorders
- Experience using Motivational Interviewing and technological skills
- Valid unrestricted independent professional behavioral health clinical license
- Must be available to work Monday through Friday, 8:00am to 5:00pm
