Licensed Discharge Planner

Job Summary

A company is looking for a Complex Discharge Planner II (Nurse or Social Work).

Key Responsibilities

• Assess, plan and coordinate safe, effective and clinically appropriate transition plans
• Collaborate with internal and external teams for clinical case review and planning
• Establish and maintain collaborative relationships with hospitals and other identified stakeholders

Required Qualifications

• Bachelor's or graduate degree from an accredited school of nursing or social work
• 4+ years of nursing or social work experience
• 1+ years of care coordination or discharge/transition planning experience
• Current state's RN license or LICSW required
• Knowledge of behavioral health impacts on physical health preferred
