Job Summary

A company is looking for a Care Coordinator, Remote in MI.

Key Responsibilities:

• Serve as primary care manager for members with high medical needs
• Engage members to complete comprehensive needs assessments and develop individualized care plans
• Coordinate with internal teams and community resources to support member care and monitor progress

Required Qualifications:

• Active, unrestricted RN licensure in the state of Michigan
• 2+ years of experience with diverse populations and complex medical needs
• 1+ years of community outreach or field work experience in a healthcare setting
• 1 year of medical case management experience
• Access to a designated quiet workspace in the home and must reside in Michigan
