Job Summary

A company is looking for a Registered Nurse Care Coach.

Key Responsibilities:

• Utilize care management software to conduct monthly calls with Medicare patients with chronic conditions
• Build rapport and coach patients on self-management strategies and health improvement
• Implement and update the Plan of Care, connecting patients with necessary community resources

Required Qualifications:

• Current, unrestricted Michigan License
• 3+ years of experience as a Registered Nurse
• Proficiency with electronic health records and web-based applications
• Self-directed with the ability to work independently
• Good with technology and eager to learn new software
