Job Summary
A company is looking for a Medication Therapy Management (MTM) Pharmacist.
Key Responsibilities
- Provide comprehensive medication management services to members primarily telephonically and face to face
- Document medication management services for care coordination with members and their care teams
- Participate in cross-functional care team meetings to improve clinical programs and enhance processes
Required Qualifications, Training, and Education
- Doctorate of Pharmacy (PharmD) or equivalent degree
- Current unrestricted Minnesota licensure to practice pharmacy
- Graduated from a college of pharmacy after May 1996 or completed an approved ACPE program
- 3+ years of related professional experience
- Strong clinical pharmacy knowledge
