Job Summary
A company is looking for a Virtual Chronic Care Management Nurse (Contractor).
Key Responsibilities
- Conduct virtual assessments and provide ongoing updates on patients' health goals
- Educate patients and families on conditions, treatment options, and self-management strategies
- Document patient interactions and care plans accurately in electronic health record systems
Required Qualifications
- Bachelor's degree in Nursing (BSN) from an accredited institution; Master's degree (MSN) preferred
- Active Registered Nurse (RN) license in good standing
- Minimum of 2 years of clinical experience in chronic care management, preferably for Medicare beneficiaries
- Proficiency with Electronic Health Records (EHR) systems
- Comfortable with technology and adapting to new virtual care platforms
