Job Summary
A company is looking for a Palliative Care Nurse Practitioner.
Key Responsibilities
- Conduct specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team
- Oversee interdisciplinary case conferences to create and monitor individualized care plans for members with advanced serious illness
- Provide education on palliative care to Community Health Workers and Oncology Nurses, focusing on symptom management and advance care planning
Required Qualifications
- At least 3 years of nurse practitioner experience, with 2 years in palliative care, preferably in outpatient oncology
- Advanced Certified Hospice and Palliative Nurse Practitioner (ACHPN) certification
- Unrestricted nurse practitioner license with a willingness to obtain additional state licenses
- Experience in a startup or tech-forward company is preferred
- Familiarity with technology and virtual care platforms
