Ohio Licensed RN Care Manager

Job Summary

A company is looking for an RN Care Manager to oversee patient care plans and collaborate with field staff.

Key Responsibilities

• Review and revise the Plan of Care (POC) to ensure accuracy and compliance with regulatory standards
• Participate in interdisciplinary team meetings and manage healthcare costs through effective care decisions
• Assess patient clinical, functional, and behavioral health status for timely follow-up and service allocation

Required Qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review preferred
• Two years of case management and/or utilization management experience preferred
• Knowledge of Medicare, Medicaid, and compliance guidelines for patient clinical care
