Job Summary
A company is looking for a Social Worker in Population Health.
Key Responsibilities
- Provide psychosocial assessments and develop comprehensive treatment plans for patients and families
- Participate in interdisciplinary treatment teams and provide consultative services to healthcare team members
- Facilitate group therapy, individual therapy, and assist with discharge planning and community resource referrals
Required Qualifications
- Master's Degree in Social Work or related field
- Relevant state licensure (LGSW, LCSW, LISW, etc.) based on location
- Two years of social work experience
- Certification in Case Management preferred
- Ability to obtain Master's-level licensure in Ohio within 6 months of hire
