A company is looking for a Health Coach - Diabetes.
Key Responsibilities
- Assist members by completing health assessments and implementing care plans for chronic conditions
- Provide telephonic support and coaching to improve members' health and lifestyle
- Monitor wellness plans and engage with healthcare providers to align care plans
Required Qualifications
- Associate's degree with 2 - 4 years of related experience
- Certification as a Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, or Master's in Behavioral Health
- Health Coach Certification is required
- Residency in Oklahoma is required
- CDCES certification is preferred
