Job Summary

A company is looking for a Patient Care Advocate.

Key Responsibilities:
  • Close care gaps and improve member and provider experience through outreach and face-to-face interactions
  • Collaborate with providers to improve HEDIS measures and coding
  • Schedule doctor appointments, conduct telephonic outreach, and provide education to members regarding care gaps

Required Qualifications:
  • Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field
  • 2+ years of work experience in direct patient care, social work, quality improvement, or health coaching in a managed care environment
  • Total of 4 years of work experience required for the position
  • Experience in resolving deficiencies impacting plan compliance to meet State and Federal standards for HEDIS
  • Experience in supporting the implementation of quality improvement interventions and audits in relation to plan providers
