Job Summary
A company is looking for an RN Care Coordinator for a remote contract opportunity.
Key Responsibilities
- Perform utilization management for inpatient admissions and evaluate discharge planning needs
- Collaborate with healthcare providers to ensure appropriate care settings and maintain quality
- Identify quality of care issues and refer cases for further evaluation and management
Required Qualifications
- Active nursing license in PA, NJ, or DE
- Residency in the tri-state area
- Must have own computer and monitor
- 3+ years of acute care clinical experience in a healthcare setting
- Desirable experience in discharge planning and/or utilization management
