Job Summary
A company is looking for an LPN Care Coach.
Key Responsibilities
- Educate patients on self-management skills and goal setting for chronic conditions
- Implement and improve the Plan of Care, including medication updates and care coaching
- Conduct Transitional Care Management activities to reduce hospital readmissions
Required Qualifications
- Current, unrestricted Compact LPN license
- Proficiency with electronic health records and web-based applications
- 5+ years of experience as a Licensed Practical Nurse
- Ability to work independently and meet performance metrics
- Availability to make calls on weekdays during specified hours
