Job Summary

A company is looking for a Transition of Care Coordinator - Eastern Time Zone.

Key Responsibilities

• Engage and enroll patients through telephonic outreach, serving as their primary contact
• Develop and implement comprehensive care plans in collaboration with the interdisciplinary care team
• Identify and address social determinants of health, connecting patients to necessary resources and support

Required Qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Strong knowledge of community resources
• Intermediate skills with MS Office Suite, including Outlook and Teams
• Ability to work effectively in a remote environment with required internet speeds
