Transition of Care Nurse - AZ Licensed
Job Summary
A company is looking for a Transition of Care Nurse (remote - AZ).
Key Responsibilities
- Contact members within 3 days of discharge to ensure safe discharge and follow-up
- Facilitate completion of all discharge orders and conduct TOC surveys
- Perform follow-up with members up to 30 days post-discharge based on LACE tool scoring
Required Qualifications
- 2 years of nursing experience in a healthcare setting
- Associate's Degree in Nursing
- Current, active, unrestricted nursing license in the state of Arizona
- Prior HRA experience is preferred
- Bachelor's Degree in Nursing is preferred
