Ensure timely discharge planning and communication

Qualifications:

Associate's degree in Nursing preferred
Minimum 2 years of experience in medical management
Working knowledge of MCG, InterQual, and NCQA standards
Active and Unrestricted LVN license
Certification Managed Care Nursing (CMCN) preferred
