Virginia Licensed Transition Care Coordinator

Job Summary

A company is looking for a Transition Care Coordinator/RN/LCSW/LPC/LPN.

Key Responsibilities

• Facilitate safe and effective transitions in collaboration with care teams and facilities
• Participate in interdisciplinary care team meetings
• Coordinate care for individuals, particularly those with Medicaid or CCC Plus populations

Required Qualifications, Training, and Education

• Associate's, Bachelor's, or Master's degree in Health & Human Services or related field
• Licensed as RN, LPN, LCSW, or LPC in the state of Virginia
• Minimum of two years of case management or long-term care experience
• Experience in nursing, social work, or human services field
• One year of experience working with Medicaid or CCC Plus populations
